This patient presented requesting an altered nose shape with a smaller, straighter bridge and was also unhappy with the shape of her tip. The tip tended to be droopy and curled down when she smiled. An additional challenge was the nostril rims were retracted and sitting too high relative to the underside of the nose ( columella ) giving her nostrils a large appearance. This is an extremely difficult rhinoplasty problem.
After surgery the nostril rims have lowered, the nose is deprojected with a straighter profile and the tip position is addressed.
